Summary

We are seeking an experienced Registered Nurse with demonstrated clinical and management leadership skills to join our dedicated team of multi-disciplinary palliative care experts. You will act as a resource and actively participate in quality improvement methodology to ensure a high standard of patient care.

This position will suit an energetic nurse who has a passion for providing evidenced based palliative care and has the ability to lead a team and coach staff to deliver an exceptional patient and family experience.

The successful candidate will be a team player, use a strength based approach to leadership and work closely with other ANUM's and Coordinators. Our dedicated Palliative Care Nurse Educator works 0.7FTE and is available for ongoing and hands on educational support. You will report to, and be supported by, the Palliative Care Nurse Manager.

About the department:

Cabrini specialist palliative care inpatient unit provides care to patients admitted for assessment and management of complex symptoms and end of life care that cannot be managed in the community or in the absence of a multidisciplinary team.

Key responsibilities:

  • Coordination of the shift and planning for the forthcoming shift and patient activity. 
  • Provides clinical expertise for the clinical team. 
  • Communicating and escalating issues to appropriate senior staff as per policies and procedures and ensuring follow up actions. 
  • Providing support and coaching staff when required to assist in problem solving. 
  • Liaising with medical staff and other health professionals to facilitate an integrated model of care. 
  • Triage community calls documenting clinical information and outcome. 
  • Liaise with the Homecare team re: out of hours visits and admissions.

Selection Criteria

The successful candidate will have:

  • Post-graduate qualifications and/or previous experience in Palliative Care or relevant stream
  • Hold or working towards, a Postgraduate qualification
  • Registered with APHRA as a Registered Nurse
  • Demonstrated leadership skills with proven ability to motivate a team
  • Quality Improvement / Auditing experience with an interest in driving quality improvement activities and ensuring outcomes are delivered.
