Palliative Care South East provides palliative care in the southeast metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support at home to improve the quality of life of individuals, carers and families. Our specialist palliative care services are provided to people of all ages, free of charge, in the Local Government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.

Working within an interdisciplinary team, the Community Palliative Care Nurse will manage palliative symptomatic issues holistically, providing direct and/or consultative specialist palliative care for clients and their families/carers in their homes. The Community Palliative Care Nurse will participate in care coordination and case management in partnership with appropriate external stakeholders or community service providers as requested. The Community Palliative Care Nurse will display high levels of professional behaviours at all times and promote a positive workplace culture.

Selection Criteria

Required Skills and Experience

  • Registered Nurse (Div 1) 
  • Current registration with AHPRA 
  • Experience in palliative care or related field 
  • Current Victorian Drivers Licence
